Job Title: Palliative Care Nurse Practitioner

Schedule: M-F, 8-4pm (minimum 32 patient contact hours required weekly)

Salary: 100,000-120,000 (base for year one, reduced base with a productivity upside year two)

Location: Based in Helena, MT. Some call coverage in Butte, MT (Hybrid – mix of in-office and remote work)

In this rewarding role, you'll provide heartfelt palliative and home-based primary care (HBPC), helping patients and families navigate advanced illnesses with empathy and expertise. Working closely with an interdisciplinary team, you'll focus on easing symptoms, honoring goals of care, and improving quality of life every day.

Key Responsibilities

  • Deliver compassionate palliative care consultations in line with policies, guidelines, and Montana regulations, under physician collaboration.
  • Perform thorough history and physical exams, interpret labs and diagnostics, and document findings thoughtfully in patient records.
  • Create personalized care plans to manage complex symptoms, prescribe medications/treatments, coordinate referrals, and facilitate smooth transitions to hospice when appropriate.
  • Guide patients and families with clear, caring education on disease progression, care options, and what to expect.
  • Collaborate with the interdisciplinary palliative team to ensure holistic symptom control and the best possible support.
  • Support patients and families in exploring and documenting advance care directives.
  • Consult with your collaborating physician on any care plans outside standard protocols.
  • Build warm relationships with team members and community partners to help grow our program.
  • Maintain accurate, timely documentation, including clinical notes, assessments, orders, and proper coding for visits.
  • Serve as a trusted resource, offering education and guidance to the team and community providers.
  • Help identify and enroll eligible patients in chronic care management programs.
  • Perform other duties as needed to support our shared mission.

Qualifications

  • Graduate of an accredited nursing school and Nurse Practitioner program.
  • Active Montana NP license, CPR certification, and current DEA license.
  • Masters degree plus at least 1 year of Nurse Practitioner experience and 1 year in hospice, palliative care, or family practice.
  • Chronic care management experience preferred.
  • Valid Montana drivers license with a reliable, insured vehicle.
  • Strong observation, communication (verbal/written), problem-solving, and clinical skills, with a genuine heart for compassionate care.
